Wedding Order Form

This is an order form for Basket and Boxes Wedding items, we will need either an email address or a phone number (preferably both) in case there are any questions about your order. We will contact you within 24-48 hours with a price total including shipping (if you do not hear from us please resubmit your order). We do not accept credit cards at this time. Please submit a check or money order made out to Monica Harris to the address on the home page. Once we receive your check, we will generally ship your order within 10 days. Thank you.
